Quirks & Gotchas
Fees
- Initial application fee: $400** regardless of pathway (direct or FCVS)
- Credentialing fee adds $300 if applying direct to NMMB ($700 total), or $375 FCVS fee** if using FCVS ($775 total) — FCVS actually costs more
- Fingerprint/background check fee: $45.25**, paid by applicant, separate from application fee; applications will not be processed without it
- Renewal fee:** Sources conflict — one cites $200 (biennial), another cites $600 (triennial). The administrative code establishes a triennial cycle; verify the current amount directly with the board
- Telemedicine license: $400**, renewable on the same July 1 cycle as standard licenses
Fingerprints & Background Check
- Two full sets of fingerprints required** (not one), plus a completed fingerprint certificate form, signed authorization, and fee — all submitted at time of application
- Board will not begin processing without the fingerprint package
- Licensing can be granted while screening is still pending, but if a felony or Medical Practice Act violation turns up afterward, applicant must submit legal documents for board determination
- Long list of disqualifying crimes including DUI, fraud, false statements under oath, tax evasion, and crimes against children
Jurisprudence Exam
- No standalone jurisprudence exam.** New Mexico does not require a separate jurisprudence test
- The equivalent is a mandatory 1-hour CME credit per renewal cycle earned by self-reviewing the New Mexico Medical Practice Act and board rules, with physician attestation at renewal — it is not proctored, but it is auditable
Other Gotchas
- Maximum 6 attempts per USMLE step**; COMLEX attempts are unlimited
- 7-year window** to complete the entire USMLE sequence from the date the first step was passed — if you exceed it, the clock restarts
- MD/PhD exception:** 10-year window
- Board may grant case-by-case extensions for military service, documented disability, or extreme hardship
- Passing score minimum is 75 on each component — scores cannot be averaged across steps
CME & Mandatory Training
- 75 hours total per 3-year cycle**, Category 1 AMA PRA credit
- 1 mandatory hour: New Mexico Medical Practice Act review** (self-study, physician attestation at renewal)
- 5 mandatory hours: Pain management and prescribing of controlled substances** — only required if physician holds both a federal DEA registration and a New Mexico controlled substance registration
- 1 mandatory hour: Nutrition** — effective July 1, 2026 (counts toward the 75-hour total)
- No proctored jurisprudence exam, but the practice act review is auditable; non-compliance can result in a $500 fine plus a reportable letter of reprimand
- CME records must be retained for 1 year after completion and can be audited at any time
- CME window: July 1 through June 30 before the triennial renewal date
Renewal Oddities
- Pro bono reporting required at renewal:** Physicians must report either 50 hours of pro bono service or a $500 donation to civil legal service providers annually — this is a reporting/attestation requirement, not enforced with a hard penalty, but it appears on the renewal form and is frequently overlooked
- Malpractice insurance minimum must be reported:** $200,000 per occurrence / $600,000 annual aggregate — physicians must attest to coverage or exempt status; no coverage = renewal blocked
- License renews on July 1 triennially; first license may be valid only 1–13 months depending on when it is issued, essentially making the first cycle very short and the first renewal surprisingly early
Timeline
- Target processing time: 3–4 months (not guaranteed, not statutory)
- Expedited license (requires current ABMS board certification, 3 years active practice, clean disciplinary history): issued within 30 days of a complete application**
- Applications are valid for 1 year from receipt; incomplete applications trigger a 30-day cure notice — miss it and the application may be closed
- Background check does not hold up licensure, but a clean screen is required before the license is considered fully unencumbered
Application Requirements
- Two physician recommendation forms** from physicians with personal knowledge (not just references — must be personal)
- Work experience and hospital affiliation verification for the last 2 years** required
- 3 years of malpractice history** verification required
- International medical graduates: valid ECFMG certificate required; no exceptions
- Passport-quality photo taken within 6 months of application — older photos rejected
- If direct-applying (not using FCVS), exam score transcripts must be sent directly from the testing agency; self-submitted copies not accepted
Other Gotchas
- A licensed physician may supervise no more than 2 physician assistants in most settings — exceeding this requires explicit board approval and is limited to publicly-funded or charitable facilities
License Duration Trap
- Standard license initial term can be as short as 13 months if issued in June, putting the first renewal due almost immediately after the license arrives
- Expedited and telemedicine licenses are valid up to 12 months only, then require conversion to a standard license or renewal
Interstate & Compact
- New Mexico is an Interstate Medical Licensure Compact (IMLC) member — expedited pathway available if another state is the state of principal license
- Independent practice: no collaborative agreement required with NPs or other mid-levels (beyond the 2-PA supervision cap)
